This page explains the paperwork process. It is not a participation waiver, rental contract, or payment authorization, and reading it does not sign any agreement.
Review the policies for your activity
Before registering or booking, review the fees, payment schedule, cancellation and withdrawal rules, refund or credit policy, attendance and makeup rules, and any program-specific commitments provided for that activity. Ask the office about anything unclear before accepting or paying.
This website guide does not create new fees, refund promises, cancellation deadlines, or changes to an agreement you have already signed.
Participation paperwork
The parent or legal guardian of a minor, or the adult participant, should review and complete the applicable participation documents. These may include an athlete waiver, emergency and medical information, and program or team agreements. Ask the office to confirm what is required for your activity.
Open your iClassPro account and complete any required agreements presented there. If a document is missing, cannot be opened, or cannot be saved, contact the office for the correct document and a copy. New RebelzOS signing forms are not open during the website-only launch.
Rentals, parties, and outside groups
Contact the office to confirm the booking, applicable charges, cancellation terms, and required rental or participant paperwork. An inquiry is not a confirmed reservation. An athlete waiver and a rental agreement serve different purposes; ask which documents your booking requires.
Signing and keeping a copy
Read the complete agreement before signing, confirm that the participant and signer details are correct, and review any electronic-signature disclosure. Keep a copy of the completed agreement. Contact the office for help obtaining a copy, correcting an error, or arranging an alternative signing method.
A contact-form submission, imported customer record, or visit to this page is not proof that a required agreement has been signed.
